Remote Clinical Optimization Pharmacist

Position Summary

The Clinical Optimization Pharmacist serves as a key member of PACE and/or Hospice interdisciplinary care team, providing comprehensive medication management and clinical support to patients, prescribers, nurses, and caregivers via a centralized care center. This pharmacist leverages clinical expertise, technology, etiquette, and excellent communication skills to optimize medication therapy, ensure regulatory compliance, and enhance patient quality of life through safe, effective, and compassionate pharmaceutical care.

Location:  Mt. Laurel, NJ (Option for hybrid-remote after 6 months) 
Status: Full-time, 40+ hours per week

Key Responsibilities

Clinical Support & Consultation

    • Use clinical decision support software to conduct continuous precision polypharmacy reviews to enhance acute and chronic disease management and minimize adverse drug events.
    • Provide evidence-based medication recommendations to providers, nurses, and caregivers via phone, secure messaging, and telehealth or EHR platforms.
    • Perform ongoing interventions per established clinical protocols to be developed and maintained in collaboration with providers and IDT as requested
    • Review new and existing medication requests for appropriateness, safety, and compatibility with patients’ goals of care.
    • Support enrollments and admissions, care transitions, and changes in level of care.

Medication Management & Monitoring

    • Perform precision polypharmacy reviews using specialized software to identify and resolve medication-related problems.
    • Monitor high-risk and high-cost medications, drug interactions, renal/hepatic dosing, and pharmacogenomic factors when applicable.
    • Ensure alignment of medication regimens with drug formularies, established clinical protocols, and payer guidelines.

Provider & Care Team Collaboration

    • Collaborate with patients, prescribers, nurses, and care coordinators to develop individualized care plans that support comfort-focused and/or maintenance therapy goals.
    • Educate care teams on therapeutic alternatives, symptom management protocols, and evidence-based pharmacotherapy.

Care Center Operations & Technology

    • Document all interventions and communications in electronic health records (EHR) and pharmacy management systems.
    • Utilize clinical software, artificial intelligence tools, and decision-support systems to streamline clinical documentation and recommendations.

Quality & Compliance

    • Ensure all activities adhere to federal and state pharmacy laws and CMS regulations.
    • Participate in internal audits and performance improvement activities as requested.
    • Contribute to protocol and policy development for medication use, safety, and workflow optimization as needed.

Qualifications

Education & Licensure

    • Doctor of Pharmacy (PharmD) from an accredited college of pharmacy.
    • Active, unrestricted pharmacist license in New Jersey (multi-state licensure preferred).
    • Board Certification in Geriatric Pharmacy (BCGP) or willingness to obtain within first four months of hire (paid upon completion by GalenusCare)
    • Completion of an approved pharmacogenomic certification course prior to or within four months of hiring (also paid)
    • Completion of two additional GalenusCare University courses in Collaborative Practice (State Specific) and GalenusCare’s proprietary Clinical Decision Support platform.

Experience

    • Minimum three years of experience as a clinical pharmacist in hospice, palliative care, geriatrics, or chronic care management in ambulatory or in-patient settings.
    • Prior experience in a call center or remote clinical support setting preferred.
    • Familiarity with EHR systems and clinical decision-support software.
